Building Your Enrollment Form

With our online enrollment system, centers can set up their enrollment form, which will then display any question(s) they'd like parents to answer. Follow the instructions below to get started with creating your enrollment form in Kangarootime.

Building Your Enrollment Form in Kangarootime

1. Select Enrollment
2. Select Settings
3. Select Enrollment Forms
4. Select Add Form to create your form
   • You can have 1 form per center

Creating & Managing Sections on Your Form

1. Select Add New Section
2. Add your Section Title (example: Child Medical Information)
   • Ex: When adding questions to this form around medical information & immunizations, you can set those questions to display within this section
   • Make sure you have Show Selection Title checked if you'd like this section to appear on the parent's enrollment form
3. To move sections around, select the 4 arrow icon and drag the section to your desired spot

Creating & Managing Pages on Your Form

1. Select Manage Pages
   • Default pages are Children & Primary Contacts
2. Select Add Page
   • The Secondary Contact page allows you to collect authorized contact information and require parents to provide emergency contacts
   • Customer Defined page(s) allow for you to spread out questions onto pages customized by you. These pages will have account-specific questions on them.
3. To re-order your pages, select the 4 arrow icon and drag pages into the order of your choosing.

Adding Questions to Your Enrollment Form in Kangarootime

1. In your Enrollment Form, Select Add New Question. Depending on which page you're on, you will either be adding or creating questions to that specific page (Ex: Children's Page, where all information is related to child(ren) on the account)
2. Select System Fields to add pre-created questions to your form. Answers to these questions will be added to the corresponding fields in the system (Ex: a child's medical conditions will be saved within their Health & Wellness record
3. Select Custom Fields to add any custom questions you've created 
   • For more information on creating Custom Fields (Questions), please read this article
4. Once questions have been added, they can be re-arranged, made required, or organized by section on your page

Creating a Link for Your Form

1. From the Enrollment tab on your navigation bar, select Settings
2. Within the Web Links tab, select Create
3. Give your Link a name (this is for internal only), and affiliate it with your center(s)
   • Note: you will need one link for each form you have.
   • If your form is the same across center(s), you can affiliate one Link with each center & parents across your locations will see the same form (but they will first be prompted to choose their center).
